I am Patti Sexton, a licensed social worker with graduate coursework in gerontology. Over the past fifteen years, I have gained extensive experience working with waiver programs as a case manager, care coordinator, and support planner. I am passionate about collaborating with individuals of all ages and abilities, along with their families or support teams, to create personalized, comprehensive support plans designed to focus on what is meaningful and important to each person. Together, we develop tailored strategies for assistance and services that meet the individual’s needs, enabling them to live well, live safely, and thrive in their chosen environment.

I started Silver Lining to ensure that individuals and families have assistance navigating complex waiver programs. My background in gerontology and extensive work as a case manager and support planner drives my passion for creating personalized plans that prioritize what is truly meaningful to you.
